1. This plan provides discounts at certain healthcare providers for medical services

2. This plan does not make payments directly to the providers of medical services

3. The plan member is obligated to pay for all healthcare services but will receive a discount from those healthcare providers who have contracted with the discount plan organization

4. This discount card program contains a 30 day cancellation period.

5. FL, LA, ND, OK, SC, SD and TX residents: Member shall receive a full refund of membership fees, excluding registration fee, if membership is cancelled within the first 30 days after receipt of membership materials.

6. AR and TN residents: A refund of all fees will be issued if membership is cancelled within the first 30 days.

7. MD Residents: The membership fee and one-time registration fee (minus $5.00) will be refunded if cancelled within the first 30 days and upon return of the discount card.

11. The range of discounts for medical or ancillary services provided under the plan will vary depending on the type of provider and medical or ancillary service received.

12. The discount medical card program makes available, before purchase and upon request, a list of program providers, including the name, city, state, and specialty of each program provider located in the cardholder’s service area.

13. Refund Policies and program Terms & Conditions must be provided to a prospective member at the time of application.  

14. Pharmacy discounts are Not Insurance, and are Not Intended as a Substitute for Insurance

15. The Pharmacy Drug Card discount is only available at participating pharmacies.

16. TelaDoc™ is not available to residents or in the state of Oklahoma.

17. TelaDoc™ does not prescribe DEA controlled substances or lifestyle drugs.

18. TelaDoc™ provides services for members 10 years of age or older.

19. TelaDoc™ does not replace your primary physician. 

20. Dental Benefit is not available to VT residents.

21. Chiropractic Benefit is not available to VT residents.

22. Vision Benefit is not available to VT residents.
